چکیده
Abstract Aims Our centre routinely uses ultrasound in adults to confirm diagnosis of appendicitis. This study investigated its use the suspected appendicitis children and impact on management. Methods Retrospective cohort (5-16 years) managed by general surgeons with during 2019. Primary outcomes were use. Secondary diagnosis, length stay, complications negative appendicectomy rate. Results A total 193 admitted The majority underwent an scan (169/193 [88%]) which identified inflammed appendix or secondary-inflammation nearly all cases (65/71 [91%] 5/71 [7%] respectively). normal was more than half not diagnosed (53/95 [56%]), rest showing ovarian pathology (3/95 [3%]) (39/95 [41%]). Ultrasound had high sensitivity (92% [CI 83-97%]) specificity (100% 96-100%]) for treated laparoscopic (75/85 a minority medically-managed (10/75 [12%]). One mass medically-managed, two Meckels Diverticulitis small bowel resection. Negative rate extremely low (2/77 [2.5%]). Post-operative complication (6%). Median stay short other diagnoses (2days [IQR 2.2] 1day 0] Discussion pre-operative ultrasound, highly sensitive specific low, likely related routine imaging.

Negative appendicectomy rate as a quality metric in the management of appendicitis: impact of computed tomography, Alvarado score and the definition of negative appendicectomy
INTRODUCTION The negative appendicectomy rate (NAR) is a quality metric in the management of appendicitis. While computed tomography (CT) has been associated with a low NAR, Alvarado scoring produces an acceptable NAR. The definition of negative appendicectomy may affect the NAR. This study examined the impact of CT, Alvarado score and definition on the NAR. METHODS The charts of 1,306 emerg...
متن کاملImportance of clinical assessment in diagnosis of acute appendicitis and its role in decreasing negative appendicectomy rate.
BACKGROUND Appendicitis is a common diagnosis, but is by no means a simple one to establish. It is impractical to have a definitive preoperative diagnosis, which leads to an appreciable rate of negative appendicectomy as reported in world literature varying from 20-40% with its morbidity around 10%. This retrospective study investigated the value of clinical assessment and medical imaging (ultr...
